As a Licensed Clinical Professional Counselor, you will:

• Provide comprehensive mental health assessments and conduct detailed patient intakes focused on diagnostic and functional evaluations.
• Work collaboratively with primary care providers to devise and implement effective treatment goals and plans, ensuring alignment with the agency's protocols and Federally Qualified Health Center (FQHC) guidelines.
• Deliver brief, focused therapeutic interventions for individuals in need of mental health support.
• Identify patients requiring specialty mental health facilities and advise on appropriate referrals.
• Monitor patient progress through regular follow-ups, adjusting treatment strategies as needed.
• Develop tailored relapse prevention plans for at-risk patients to promote stability and recovery.
• Engage in comprehensive documentation of patient progress in aligned treatment charts, ensuring all interventions are accurately recorded.
• Educate patients on self-management strategies, empowering them to take an active role in their recovery process.
• Maintain effective communication with primary care providers and other involved parties to ensure comprehensive treatment for the patient.
• Participate in program evaluations and contribute ideas to enhance service outcomes and patient experiences.

Your work will primarily focus on children, adolescents, and adults facing mental health challenges and psychosocial issues. By implementing evidence-based practices and fostering strong therapeutic relationships, you will play a critical role in enhancing their emotional, physical, and mental well-being. To excel in this role, you must possess:

• A minimum of a Master’s Degree in Clinical Counseling.
• A current and valid Licensed Clinical Professional Counselor (LCPC) license.
• A solid understanding of mental health assessment, treatment planning, and various therapeutic modalities, including family therapy and group therapy.
• Expertise in developmental psychopathology and diagnostic criteria as outlined in the DSM-IV.
• Exceptional organizational skills to maintain clinical records and prepare necessary reports.
• Ability to work effectively under pressure and manage multiple patient needs collaboratively.

Preference will be given to candidates with:

• Proficiency in English and Spanish, enhancing communication with our diverse patient base.
• Experience in outpatient mental health settings, particularly in integrated care models.
• Previous work with underserved communities and an understanding of community resources available to support vulnerable populations.

We offer a competitive salary ranging from USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
