• Conduct comprehensive mental health assessments that consider the interplay between patients' emotional and physical well-being.
• Develop integrated treatment plans focusing on both psychiatric and medical aspects to ensure a comprehensive approach to patient care.
• Facilitate individual and group therapy sessions using evidence-based practices tailored to meet the specific needs of each patient.
• Work in partnership with medical providers to deliver coordinated care that targets effective treatment outcomes for our patients.
• Provide crisis intervention services as needed, offering immediate support to patients in mental health emergencies.
• Educate patients and their families about the connections between mental and physical health, empowering them to actively participate in their treatment.
• Maintain accurate records and confidential documentation in adherence to relevant legal regulations and ethical guidelines.
• Actively participate in regular team meetings and interdisciplinary case conferences to discuss treatment plans and patient progress.
• Stay informed of the latest research, treatments, and best practices within integrated care, enhancing the quality of services provided.

You will provide care for a diverse population of patients experiencing mental health challenges in conjunction with medical conditions. This role requires a refined sensitivity to the complexities that arise in co-occurring conditions, such as those faced by patients dealing with chronic pain or substance use disorders. Your therapeutic interventions will be crucial in navigating their integrated care needs and enhancing their quality of life.

• A Master’s or Doctoral degree from an accredited institution in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology.
• Current and valid licensure as an LCSW, LPC, or LMFT in the state of Arizona.
• Proven experience addressing the mental health needs of individuals with co-occurring medical conditions.
• Strong collaborative skills, particularly in working alongside medical professionals to deliver an integrated care approach.
• Excellent communication and interpersonal skills, with the ability to build rapport and maintain professional boundaries.
• A commitment to ethical practice and patient confidentiality, ensuring compliance with both legal and organizational standards.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Demonstrated capability to work effectively as part of an interdisciplinary team.

• At least two years of clinical experience in a mental health counseling role, specifically in integrated or collaborative care settings.
• Previous involvement in telehealth services or remote therapy environments.

This position offers a competitive salary range between $75,000 to $100,000 annually, commensurate with experience and qualifications.
