This position is primarily remote, allowing you to maintain a flexible work-life balance while serving patients in need from the comfort of your home office. Our team schedules are designed to accommodate both patient needs and therapist availability. Monday through Friday, you will typically work from 8:00 AM to 5:00 PM, with occasional evening shifts to better serve our patients. • Conduct comprehensive mental health assessments, considering how medical conditions affect patients' mental health and vice versa.

• Develop and implement personalized treatment plans that cater to the unique psychiatric and medical needs of each patient.

• Facilitate individual and group therapy sessions, using evidence-based interventions to address the diverse needs of patients.

• Collaborate closely with physicians, nurse practitioners, and other healthcare providers to ensure integrated and coordinated care.

• Provide support during mental health crises and emergency situations, utilizing crisis intervention techniques to maintain patient safety.

• Educate patients about the interplay between mental health and chronic medical conditions, fostering their capacity for self-management.

• Maintain thorough and confidential clinical documentation, adhering to legal and ethical standards, primarily using an electronic health record (EHR) system.

• Participate in team meetings and care planning discussions to enhance treatment strategies and collaborate on complex cases.

• Stay informed on contemporary research, best practices, and methodologies in integrated care to continually improve your practice.

You will work with a diverse patient population, particularly those grappling with dual diagnoses of mental health and chronic medical conditions. Our patients often experience layered complexities arising from issues such as chronic pain, substance abuse, and emotional distress. As a therapist, your expertise will be essential in not only addressing their mental health needs but also understanding their overall physical health challenges, creating a personable and empathetic treatment approach that fosters trust and healing.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Possession of a current and valid state licensure as an LMFT, LPC, or LCSW.

• Demonstrated ability to address the mental health needs of individuals with co-occurring medical conditions.

• Prior experience delivering integrated care within an interdisciplinary team setting is highly valued.

• Excellent communication and interpersonal skills that foster a good rapport with patients and colleagues alike.

• Capacity to maintain professional boundaries and adhere to ethical guidelines while ensuring patient confidentiality.

• Familiarity with electronic health record (EHR) systems is preferred, alongside proficiency in standard computer applications.

• At least one year of direct experience in a clinical mental health setting, with a focus on integrated care.

• Background in working with chronic pain management or substance use disorders could enhance your candidacy.
