• Develop and implement tailored support packages designed to assist clients in overcoming their specific challenges, including addiction and mental health disorders.
• Conduct comprehensive assessments of clients’ mental, emotional, and social needs through virtual interviews and utilizing standardized assessment tools.
• Establish measurable and individualized goals for each client, monitoring progress through regular follow-ups and adapting treatment plans as necessary based on client feedback and outcomes.
• Provide guidance, psychoeducation, and counseling during sessions to empower clients in choosing the best action plans for their recovery.
• Document client interactions and maintain up-to-date records in compliance with legal standards and organizational policies, ensuring confidentiality and privacy.
• Participate in case conferences alongside medical and rehabilitation professionals to coordinate comprehensive care strategies for clients.
• Act as a key liaison between clients and community resources, making referrals as needed to support ongoing recovery outside of therapy sessions.
• Stay informed on best practices and innovations in the social work field through continuous professional development opportunities.

Your primary focus will be supporting individuals navigating the complexities of mental health issues post-detoxification. This population often faces co-occurring disorders and may require a multifaceted approach to treatment that addresses both psychological and social factors. With a deep understanding of addiction and recovery frameworks, you will help clients maneuver through their experiences while aiming to restore stability and well-being.

• A valid and active LCSW license in the State of Illinois.
• A Bachelor’s Degree in Social Work (BSW), Psychology, Sociology, or a related field is required; a Master’s degree is preferred.
• Demonstrated experience in individual therapy, crisis intervention, and case management.
• Familiarity with evidence-based therapeutic practices and a solid understanding of social theories related to mental health.
• Strong interpersonal skills with the ability to establish rapport and trust with diverse populations.
• Capacity for resilience and analytical thinking, enabling effective assessment and crisis management.
• Willingness to undergo background checks as part of the hiring process.

• Previous experience in a detox or rehabilitation setting is highly desirable.
• Knowledge in using electronic health records (EHR) systems for documentation and case tracking.
