) Vitability Health Of New Jersey is seeking a skilled and compassionate Medical Doctor to join our telehealth team. The ideal candidate will provide virtual care to patients transitioning from hospital or rehabilitation settings to their homes. This role offers the flexibility to practice medicine through telehealth while ensuring high-quality care delivery. The candidate must have a multi-state medical license, with a primary focus on serving patients in New Jersey.
Key Responsibilities:
  • Provide telehealth consultations to patients, ensuring comprehensive medical care and addressing their ongoing needs.
  • Collaborate with PAs NPs, care navigators, nurses, and other healthcare team members to create and implement patient care plans.
  • Address chronic conditions, medication management, and preventative care to reduce hospital readmissions.
  • Ensure compliance with Medicare, Medicaid, and other insurance requirements for telehealth services.
