Job description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This remote, part-time opportunity is for a California-licensed Pharmacist to support a Medication Therapy Management (MTM) pilot program serving Medicaid patients. The role involves conducting MTM consultations, providing patient counseling, and helping to launch a clinical pharmacy initiative. Candidates must reside in California and be actively enrolled as an ORP provider in California Medicaid (Medi-Cal).

Key Responsibilities:

  • Perform Medication Therapy Management consultations, assessing patient medication profiles to optimize therapy
  • Deliver patient education and counseling to promote medication adherence and safety
  • Accurately document clinical interventions and patient interactions
  • Collaborate with stakeholders supporting patient care outcomes and program success
  • Ensure compliance with state and federal pharmacy regulations

Qualifications:

  • Active and unrestricted Pharmacist license in California
  • Current resident of California to meet program requirements
  • Proven experience in Medication Therapy Management services
  • Enrollment in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) Provider is mandatory
  • Strong interpersonal and communication skills suitable for patient counseling
  • Ability to work independently and manage a flexible remote schedule effectively

Schedule:
