This full-time position offers an annual salary ranging from USD 90,000 to 105,000, depending on experience and qualifications. As a key member of our community health clinic, you will provide essential mental health support to individuals and families while collaborating closely with primary care teams to enhance overall patient care.

• Conduct thorough patient intakes to assess mental health and psychosocial issues, focusing on diagnostic evaluations to help establish treatment plans.
• Provide therapeutic interventions, counseling, and client education, aiming to support patients in their journey to improved health.
• Collaborate with primary care providers to monitor patient progress and adjust treatment strategies as necessary, ensuring the best possible outcomes.
• Deliver urgent mental health interventions for patients in crisis, ensuring immediate support is available.
• Maintain accurate and comprehensive documentation of patient interactions, progress, and treatment plans in accordance with agency compliance guidelines.
• Participate actively in team meetings to discuss patient care strategies, outcome evaluations, and the development of new programs.
• Advise medical staff on referrals to specialized mental health services when appropriate, ensuring patients receive comprehensive care.
• Support patients in developing self-management skills and coping strategies, empowering them to take an active role in their mental health journey.
• Engage in continuous education and training to stay informed of best practices in mental health care.
• Adhere to the Clinical Professionals' Code of Ethics, ensuring all patient interactions uphold the highest standards of professionalism.

You will primarily engage with a diverse patient population from the Erie, Pennsylvania area, addressing a spectrum of mental health needs. Your work will support individuals facing various challenges, including anxiety, depression, trauma, and substance use disorders. You will play a crucial role in promoting mental wellness, working within a team that values holistic approaches and community-oriented care.

• A Master’s Degree in Clinical Counseling or a related field is required.
• A valid LPC (Licensed Professional Counselor) license is mandatory.
• Demonstrated understanding of mental health evaluation, treatment planning, and the diagnostic criteria as outlined in DSM-IV.
• Expertise in various therapeutic modalities, including individual and group therapy, is highly beneficial.
• Excellent communication skills in English; proficiency in Spanish is a plus but not required.
• Strong analytical skills that allow for tailored assessment and intervention.
• Ability to navigate complex social dynamics affecting patient health outcomes.

• Prior experience in a similar clinical setting, particularly in a community health clinic or a Federally Qualified Health Center (FQHC), is preferred.
• Experience in managing interdisciplinary care teams and working collaboratively with healthcare professionals.
• Exposure to a diverse patient demographic, particularly in the Erie area, with an understanding of community resources.
