As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive assessments of clients experiencing major mental illnesses and severe emotional disorders.
• Creating and implementing integrated care plans that address individual needs and facilitate access to necessary services, including family and community resources.
• Providing individual, group, and family therapy tailored to clients’ presenting problems and requests for service.
• Crisis intervention when necessary, ensuring clients receive immediate support during critical situations.
• Maintaining up-to-date client records and ensuring timely completion of reports and data submissions.
• Participating in team meetings and committees to enhance agency service delivery and quality improvement initiatives.
• Providing on-call coverage during and after program hours to support client needs as they arise.

Your primary focus will be on patients in a hospice setting, many of whom face complex mental health challenges related to chronic illnesses and end-of-life issues. This role demands sensitivity and understanding, as you will work with diverse clients from various cultural backgrounds, ensuring that their unique beliefs and experiences inform their treatment plans. Your ability to build trusting relationships will be crucial in fostering a supportive environment for both clients and their families.

To excel in this role, candidates must possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• A valid state licensure as a Clinical Social Worker (LCSW).
• A strong foundation in various therapeutic modalities and best practices in mental healthcare.
• Exceptional communication and interpersonal skills essential for working with clients and collaborating with colleagues.
