• Conduct comprehensive and ongoing assessments to identify and address the needs of patients effectively.
• Maintain open communication with supervisors, direct care teammates, family members, and relevant stakeholders about individual progress.
• Develop and share a detailed schedule for appointments, home visits, and observations to monitor client progress continuously.
• Coordinate essential home visits aimed at fulfilling observatory requirements, aiding in chart reviews, interviewing staff, and conducting data analysis.
• Ensure the successful implementation of individualized program plans, while regularly reviewing their effectiveness and making necessary adjustments.
• Actively participate in Interdisciplinary Team Meetings and other relevant discussions, contributing insights and expertise.
• Prepare and complete monthly and quarterly progress notes, ensuring accurate records are maintained.
• Organize training initiatives for staff to ensure they are fully competent in implementing program plans and relevant documentation.
• Provide consistent follow-up support through interviews, home visits, and observations, addressing any evolving needs of the individual.
• Serve as an effective liaison with the public and the broader mental health community to further support our organizational goals.
• Assist in the creation and refinement of training materials aimed at enhancing our programs for those we serve.

This position allows for interaction with a diverse patient population, including adults and youth experiencing various mental health issues such as anxiety, depression, trauma, and relationship challenges. Your expertise will directly affect our clients' recovery journeys, providing critical support that fosters resilience and personal growth.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active, unrestricted licensure as a Professional Counselor (LPC) in Georgia is essential.
• A minimum of two years' counseling experience, preferably within a clinical setting, is required.
• Demonstrated experience or a specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly preferred.
• A robust understanding of legal and ethical standards in mental health practice is crucial.
• Strong interpersonal and communication skills, capable of establishing rapport with a wide range of clients.
• Proven ability to work collaboratively within a healthcare team is necessary.
• Proficiency in using EMR software and related technology for documentation and patient management.
