• Conduct thorough assessments of patients' needs, capabilities, and behaviors using established methods such as interviews, questionnaires, and direct observations.
• Develop individualized treatment plans by integrating social, medical, and psychological insights, aiming to achieve personalized therapeutic goals.
• Establish and monitor both short-term and long-term therapeutic objectives, aligning with evidence-based practices to ensure the highest standard of care.
• Encourage open dialogue with patients to explore their feelings and behaviors, providing a safe space for self-expression and reflection.
• Design and implement comprehensive treatment programs encompassing psychotherapy, psychoeducation, crisis intervention, and skill-building activities tailored to patients' specific needs.
• Collaborate with a multi-disciplinary team to provide supportive care, addressing diverse issues such as anxiety, depression, chronic pain management, and interpersonal relationships.
• Maintain thorough documentation of clinical services rendered, ensuring accurate records of initial evaluations, progress notes, treatment summaries, and discharge documentation.
• Engage in case consultations with other professionals to identify psychosocial issues and contribute to a cohesive treatment approach.
• Stay informed of federal, state, and local regulations to abide by legal and ethical standards in practice.
• Promote a safe and clean working environment by adhering to organizational procedures and guidelines.
• Take on additional responsibilities as assigned, contributing to the overall efficacy and operation of the healthcare team.

In this role, you will work with a diverse patient population, including individuals experiencing mental health challenges such as anxiety, depression, trauma, and chronic pain. The patients you serve may also include those navigating complex social and interpersonal issues. • Must hold a current and valid California LMFT license.
• A minimum of three years of experience in a mental health setting, demonstrating competence in therapeutic practices.
• Proficient in using essential software applications (Microsoft Word, Excel, PowerPoint) to manage documentation and reporting efficiently.
• Bilingual candidates, particularly those fluent in both English and Spanish, are preferred, enhancing communication with a broader patient base.

• Experience in an outpatient therapy setting with a transdisciplinary model is highly regarded.
• Previous involvement in integrating holistic treatment practices, including but not limited to psychoeducation and crisis intervention.
