  • Coordinate and conduct home visits as necessary, ensuring compliance with program requirements while performing detailed chart reviews and interviews.
  • Ensure that all implemented programs and treatment plans are effective and executed properly to meet clients' needs.
  • Participate actively in Interdisciplinary Team Meetings and relevant discussions to foster a collaborative treatment approach.
  • Document progress accurately through monthly and quarterly assessments, maintaining transparency and accountability.
  • Organize and conduct training sessions for team members to ensure the proper understanding and implementation of the program plans.
  • Provide continuous support by performing follow-up interviews, home visits, and observations based on individual needs.
