Key Responsibilities
  • Conduct comprehensive and ongoing assessments to identify and address the unique needs of each patient.
  • Maintain open lines of communication with supervisors, colleagues, family members, guardians, and other stakeholders regarding client progress and treatment plans.
  • Develop and submit a tentative schedule for appointments, home visits, and required observations to oversee client progress effectively.
  • Coordinate necessary home visits to fulfill in-home observation requirements, which may include chart reviews and interviews with staff and families.
  • Ensure that all program plans are implemented effectively and meet established goals for client care.
  • Participate actively in interdisciplinary team meetings and collaborate with colleagues on client cases.
  • Document client progress through detailed monthly and quarterly reports.
  • Organize and lead training sessions to ensure staff members understand the program implementation procedures and documentation requirements.
  • Conduct follow-up interviews and home visits to provide ongoing support, closely monitoring each individualโ€™s changing needs.
  • Maintain accurate and accessible records to facilitate transparent communication within the care team.
  • Serve as a liaison with the public and mental health community, supporting initiatives that promote mental health awareness and program development.
  • Contribute to the creation and improvement of training materials designed to enhance client programs and staff competency.
Patient Population / Client Focus

This role will involve working with a broad spectrum of clients, including both adults and youth.
