• Conduct in-depth mental health assessments, integrating findings from physical health evaluations to create holistic treatment plans.
• Develop and implement evidence-based treatment plans that address both mental and physical health needs.
• Facilitate individual and group therapy sessions, tailoring interventions to the specific struggles, strengths, and goals of each patient.
• Actively collaborate with physicians, nurse practitioners, and other medical personnel to coordinate care and optimize treatment efficacy.
• Provide timely crisis intervention strategies for patients during emergencies, ensuring their safety and connecting them to appropriate support resources.
• Educate clients on the interconnections between mental health and physical well-being, empowering them with knowledge to manage their overall health.
• Maintain precise and confidential documentation in compliance with HIPAA and other regulatory standards, utilizing electronic health record systems.
• Engage in ongoing professional development, remaining aware of best practices in integrated care and participating in regular training.

The clients you will serve are individuals facing diverse challenges, including those with co-occurring medical conditions that impact their mental health. This role requires a nuanced understanding of how psychological and physical health intersect, with an emphasis on developing integrative treatment plans that enhance the quality of life for patients confronting complex needs.

• A Master's or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Licensure as a Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Marriage and Family Therapist (LMFT) in New York State.
• Demonstrated expertise in working with clients with co-occurring medical conditions.
• Strong communication and interpersonal skills, with a proven ability to develop rapport with patients in therapeutic settings.
• Commitment to upholding professional ethics and maintaining patient confidentiality.
• Proficiency in using electronic health record systems and related software for documentation and care coordination.

Experience in an integrated care setting is highly desirable, particularly a background working with multidisciplinary teams to achieve comprehensive patient management. Familiarity with trauma-informed care practices and previous exposure to crisis intervention strategies will be beneficial in this role.
