• Conduct thorough psychosocial assessments through interviews and observations, as well as a comprehensive review of medical records to evaluate the emotional, social, and environmental factors affecting patient recovery.
• Provide active counseling and emotional support to patients and their families, efficiently addressing adjustment issues, anxiety, stress, and other mental health concerns.
• Collaborate with multidisciplinary team members to develop individualized, goal-oriented treatment plans that address the unique psychosocial needs of each patient.
• Assist patients in navigating and accessing essential community resources, including support groups, vocational rehabilitation services, and housing assistance tailored to their specific needs.
• Educate patients and their families about community resources and post-rehabilitation services to promote sustained recovery and independence.
• Participate actively in discharge planning processes, ensuring smooth transitions to home or other care settings.

Your primary focus will be on patients in the detoxification process who may be dealing with substance use disorders and require emotional and social support during a critical phase of recovery. You will also work with individuals facing various rehabilitation challenges, such as those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. By understanding their unique contexts, you will facilitate their rehabilitation and successful reintegration into the community, significantly impacting their overall well-being.

• A Master's degree in Social Work from an accredited institution is necessary for this role.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Demonstrated expertise in understanding adjustment disorders, grief, and coping mechanisms is essential.
• Strong communication and interpersonal skills to effectively engage with patients and team members.

• Previous experience working in rehabilitation or medical settings is highly desirable.
• A background in counseling, particularly in areas related to substance use or trauma, is advantageous.
