Bliss Reformation And Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our innovative team in a full-time remote capacity. This professional will be instrumental in evaluating and addressing the psychosocial needs of seniors, facilitating their journey towards emotional well-being and empowerment while collaborating with multidisciplinary teams.

The work setting is remote, providing flexibility while still ensuring that clients receive personalized care. The standard full-time schedule is Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required. This flexibility allows for continuous care and accommodates client needs. Additionally, on-call availability may be needed to address urgent issues or provide support during off-hours. While part-time schedules can be negotiated based on organizational needs, the commitment to quality care and client engagement remains paramount.

• Conduct comprehensive assessments of clients' psychosocial needs by evaluating emotional, mental, and social factors impacting their well-being.
• Through collaboration with clients and their families, develop personalized care plans that reflect individual goals and preferences.
• Provide counseling and emotional support to seniors and their families to facilitate transitions in life, including health changes and loss.
• Advocate for clients' rights by ensuring they have access to essential community resources and services.
• Work closely with an interdisciplinary team, including nurses, caregivers, and healthcare professionals to coordinate care effectively.
• Perform regular virtual visits to monitor clients' wellbeing, assess their living conditions, and address any concerns or safety issues.
• Facilitate communication within families to resolve conflicts and enhance the client’s support network.
• Educate clients and families about available community resources, including support groups and social programs aimed at improving engagement and quality of life.
• Maintain organized and detailed records of client interactions, assessments, care plans, and progress notes using electronic health record (EHR) systems.
• Stay informed about current best practices, policies, and regulations in social work to uphold ethical standards and compliance.

Your commitment will primarily be directed towards seniors and their families, addressing the specific challenges they face in navigating healthcare systems, managing mental health, and ensuring well-being. This role calls for interpersonal skills and empathy to provide support during significant life transitions.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant social work experience, ideally within a community center or geriatric setting.
• In-depth knowledge of social work principles, aging-related issues, and awareness of available community resources.
• Excellent skills in assessment, counseling, and problem-solving.
• Ability to demonstrate empathy and establish strong rapport with clients and their families.
• Exceptional communication and interpersonal skills for effective collaboration.
• Familiarity with electronic health record (EHR) systems and various computer applications.

Experience working with a diverse range of client backgrounds, particularly in geriatric care settings, is preferred. Familiarity with the challenges faced by seniors, including mental health issues such as depression and anxiety, is highly valued.

We offer a competitive salary range of $90,000 to $100,000 per year, commensurate with experience and qualifications.
