• Conduct comprehensive psychosocial assessments, utilizing interviews, observational techniques, and the review of medical records to deeply understand the emotional, social, and environmental factors influencing each patient’s recovery journey.
• Provide empathetic counseling and support to patients and their families, focusing on adjustment issues and mental well-being to enhance overall recovery processes.
• Collaborate closely with the rehabilitation team to develop tailored treatment plans that integrate the psychosocial needs and personal goals of each patient.
• Facilitate access to community resources, such as support groups, vocational rehabilitation programs, or housing assistance, ensuring patients receive holistic support that extends beyond our facility.
• Involve family members in the rehabilitation process by providing education about available community resources and effective post-rehabilitation services.
• Partake in discharge planning to prepare patients for transitions to home or other care settings, ensuring that all aspects of their care are addressed comprehensively.

As a Licensed Clinical Social Worker at Bridge Way Reflections Therapy, you will be working primarily with a diverse patient population that includes individuals recovering from various conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. • A Master's degree in Social Work from an accredited institution is required.
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
