• Conduct comprehensive psychosocial assessments by interviewing patients, observing behaviors, and reviewing medical records, to identify emotional, social, and environmental factors influencing recovery.
• Provide tailored counseling sessions for patients and their families, helping them navigate adjustment issues, emotional challenges, and coping mechanisms.
• Work closely with an interdisciplinary rehabilitation team to design and implement individualized treatment plans that address patients’ psychosocial needs.
• Assist patients in connecting with essential community resources such as support groups, vocational rehabilitation services, and housing assistance to promote holistic recovery.
• Educate patients and families about available resources, reinforcing strategies for post-rehabilitation sustainability and ongoing support.
• Facilitate discharge planning, ensuring smooth transition processes for patients returning home or moving to alternative care settings.

• A Master's Degree in Social Work from an accredited university.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

• Ability to work collaboratively in a multidisciplinary team environment.
