• Participate actively in discharge planning meetings to facilitate patient transitions home or to alternative care settings, ensuring continuity of care and support.

• A Master's Degree in Social Work from an accredited institution is mandatory.
• Possession of an active state licensure as a Licensed Clinical Social Worker (LCSW).
