Key Responsibilities
  • Conduct thorough assessments and develop targeted treatment plans that address both pre-crisis and crisis situations, collaborating with clients to create personalized strategies for behavioral health management.
  • Complete all documentation promptly and in compliance with regulatory standards, ensuring accurate and confidential patient records.
  • Utilize evidence-based screening tools in partnership with our clinical team to evaluate client needs and inform treatment processes.
  • Collaborate with various programs to implement positive behavior support plans, working closely with multidisciplinary teams.
  • Actively engage in licensing and accreditation processes, maintaining a high standard of professional practice.
  • Serve as an effective liaison to state agencies and community partners to enhance patient resources and services.
  • Establish and foster open lines of communication to uphold contract standards while promoting our mission within the community.
  • Engage with local, state, federal, and private organizations to promote programs and secure potential alliances and funding sources.
  • Pursue continuous professional growth through participation in conferences and training while representing Pathfinder Counseling Services at industry events.
  • Promote an understanding of our agency’s values, mission, and philosophy through daily interactions and contributions.
  • Perform additional duties as assigned by the Clinic Director, adapting to the needs of the organization.
Patient Population / Client Focus

You will work with a diverse population, providing care to individuals dealing with mental health issues, substance use disorders, and various developmental challenges. Our clients range from adolescents to adults, all requiring personalized support tailored to their unique histories and circumstances. As an LMHC, your skills will help foster recovery, improve emotional health, and empower clients to lead fulfilling lives.

Qualifications
  • A Master’s or Doctorate Degree in a behavioral health field is required.
  • Possession of a valid LMHC License issued by the state board in New York.
  • Proficiency in maintaining comprehensive clinical records and preparing necessary reports.
