6:00 PM TO 6:30 AM -PART TIME 24 HOURS
This position will be onsite at our central monitoring facility in Southfield MI
Scope of Work:
The Virtual Nurse is an experienced Registered Nurse (RN) and integrated care team member who leverages technology to collaborate with and support the needs of the bedside RN and the patient/family virtually. Two-way audio/visual technology provides the virtual nurse the ability to assist with admission and discharge activities, patient and family education, real-time patient quality and safety surveillance, proactive rounding, and staff mentoring/education. The purpose of this position is to harness the skills of an experienced RN to support and transfer knowledge to novice nurses within the organization, and to provide high-quality, remote nursing care in accordance with the established policies and procedures of Corewell Health.
  • Uses technology to provide remote nursing care to an assigned group of patients in collaboration with the bedside nurse and multidisciplinary team, in accordance with established unit policies, procedures, and protocols.
  • Assists with completion of admission history, medication history, and admission screening tools for assigned patients.
  • Remotely assesses the physical and psychosocial status of patients based upon the health history, documented physical examination findings, and diagnostic test results.
  • Monitors provider and bedside nurse assessment trends, laboratory values, progress notes, and diagnostic test results. Documents and communicates collaboratively with the bedside nurse regarding the patient condition and all nursing care provided. Supports the bedside nurse in ensuring appropriate information is relayed to the physician and multidisciplinary team.
  • Collaborates with the bedside nurse to initiate, update, evaluate, and revise the plan of care to facilitate the achievement of patient goals and expected outcomes.
  • Assists with identification of safety concerns and ensures findings are reported to the bedside nurse. Coordinates and collaborates with the bedside nurse on methods to mitigate safety concerns.
  • Collaborates with the bedside nurse to complete the patient learning needs assessment and provides patient and family education based on the assessment. Promotes optimum communication by answering patient/family questions, providing plan of care updates, and facilitating family meetings as needed. Documents patient and family communication and education provided.
  • In collaboration with bedside nurse, prepares the patient and family for discharge by providing discharge teaching and reviewing discharge instructions and recommended follow-up. Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • In collaboration with the bedside nurse and Care Management team, participates in rounding, assists with discharge planning, and identification of post-discharge needs, including discharge disposition, to facilitate the timely transition to the next level of care for the patient.
  • Provides coaching, teaching, educational material, and clinical resource support to bedside nurses.
  • Assists with patient triage and activation of the rapid response team in emergency situations.
  • Supports nursing shared governance and the nurse-driven practice model.
  • Supports system, hospital site and unit-based quality initiatives.

Qualifications
  • Required Bachelor's Degree In Nursing from an accredited program/institution
  • 3 years of relevant experience Requires 3-years of experience in Critical, Progressive Care or Emergency Department Required
  • 5 years of relevant experience In Critical, Progressive Care or Emergency Department Preferred
  • Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • At least one Certification from preferred list - Upon Hire required
  • Basic Life Support (BLS) - AHA American Heart Association preferred Or
  • Basic Life Support (BLS) - ARC American Red Cross preferred
  • Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association Upon Hire required
  • NIH Stroke Certification - NIHSS National Institute of Health Stroke Scale Upon Hire required
  • Specialty Certification - Nursing Upon Hire preferred
