Job description
Work collaboratively and support the Rehab and Pain Management provider teams and other clinic staff to provide professional nursing care to patients. Provide patient and family health education and health assessment.
Primary Responsibilities
- Working within scope of license, develop and implement nursing plan of care for patients. Communicate the plan of care to patients as prescribed by physician: test results, scheduling of specialized tests and treatment modalities, prescription drug instructions and other patient education. Document treatment in medical record appropriately.
- Using the nursing process, triage patient concerns via telephone and provide nursing advice. Receive phone calls relating to appointments, urgent care, inpatient admissions, prescription refills and test results in accordance with clinic protocols and coordinate proper disposition of each.
- Obtain, review, verify and/or complete health care forms related to home health care, referrals, medical equipment and other patient/physician needs as appropriate. Assist with managed care authorization and complete insurance and medical forms for patients or assist patients in completing the forms.
- Help maintain patient flow and workflow. Work collaboratively as a team member of the clinic staff to provide a high quality and efficient service to patients and clinic. Maintain patient confidentiality.
- Other duties as assigned.

- Excellent communication and organizational skills for managing workflow, patients, patient families, physicians and others under stressful conditions.
- Ability to assess and triage patients. Demonstrates reliability and maintains confidentiality.
Qualifications
Appropriate experience may be substituted for education on an equivalent basis.
Minimum Required Education: Associate's Degree in Nursing. Bachelor's Degree in Nursing preferred.
Minimum Required Experience: No experience required.
Preferred Experience: Previous clinic, doctor's office or urgent care experience preferred.
Required Certification/Licensure(s): Current Wisconsin RN License.
Must be avaialble to participate in-person for orientation and monthly team meetings.
Physical Requirements
Work requires occasionally lifting moderate weight materials, standing, or walking continuously.
Work Environment
Little or no exposure to hazards such as dust, fumes, or extreme temperatures, airborne or blood borne pathogens, extreme temperatures, or allergens.
Sensory Acuity
Ability to detect and translate speech or other communication required. May occasionally require the ability to distinguish colors and perceive relative distances between objects.
