Key Responsibilities
  • Conduct comprehensive clinical assessments of hospice patients, utilizing standardized tools and methodologies to gather essential information for developing personalized treatment plans.
  • Design and implement evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to each patientโ€™s unique situation.
  • Maintain meticulous clinical documentation in compliance with HIPAA regulations, ensuring accurate and timely records for each patient and treatment session.
  • Facilitate collaborative care by communicating effectively with interdisciplinary team members, including physicians, psychologists, and nursing staff, to ensure cohesive patient support.
  • Participate in case consultations and peer review meetings regularly to discuss complex cases, contributing to a culture of continuous improvement in the quality of care provided.
  • Act as a resource for patients and their families by offering education on mental health conditions, available treatments, and community services that can support their journey.
  • Stay informed about advancements in mental health practices and integrate new research into your clinical work, enhancing the efficacy of the services provided.
Patient Population / Client Focus

Your primary focus will be on hospice patients who are experiencing emotional challenges related to life-limiting illnesses. You will deliver compassionate care to individuals facing complex psychosocial issues, providing support not only to them but also to their families as they navigate the emotional landscape of end-of-life decisions. Your expertise will play a significant role in enhancing the patients' quality of life during a critical juncture.

Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two (2) years of post-licensure experience in providing clinical social work services.
  • Demonstrated ability to conduct diagnostic assessments and develop thoughtful treatment plans.
  • Strong interpersonal skills, with an emphasis on effective communication and relationship-building with diverse populations.
  • A commitment to upholding ethical practice, cultural competence, and social justice in all areas of clinical work.
Preferred Experience
  • Experience in hospice, palliative care, or end-of-life counseling is preferred.

Compensation and Benefits
