Position Overview

The Blended Healing Psychotherapy Center is seeking a dedicated and empathetic Licensed Clinical Social Worker (LCSW) to join our team in a fulfilling, full-time remote position focused on hospice care. As an LCSW, you will provide specialized mental health services, including counseling, therapy, and case management to individuals and families facing life-limiting illnesses. You will be an integral part of the interdisciplinary team, providing compassionate support to enhance the emotional and psychological wellbeing of our clients during their most challenging times.

  • Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care providers to deliver coordinated care and address biopsychosocial needs.
  • Provide crisis intervention services to clients and their families who are experiencing acute emotional distress.
  • Document all client interactions and treatment modalities promptly and precisely in compliance with organizational policies.
  • Stay informed of the latest research, developments, and best practices in social work and hospice care to continually enhance service quality.
Patient Population / Client Focus

Your primary focus will be on clients facing terminal conditions and their families, offering compassionate support through the entire continuum of care. Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited program is required.
