• Perform comprehensive assessments and formulate customized treatment plans based on client needs and goals.
• Engage in individual, group, and family counseling sessions tailored to the unique dynamics of hospice care.
• Monitor progress and adapt treatment strategies to facilitate effective transitions through grief and loss.
• Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care providers to deliver coordinated care and address biopsychosocial needs.
• Provide crisis intervention services to clients and their families who are experiencing acute emotional distress.
• Document all client interactions and treatment modalities promptly and precisely in compliance with organizational policies.
• Stay informed of the latest research, developments, and best practices in social work and hospice care to continually enhance service quality.

Your primary focus will be on clients facing terminal conditions and their families, offering compassionate support through the entire continuum of care. You will help clients navigate the emotional challenges that arise during end-of-life processes, including grief, anxiety, and the complexities of preparing for loss. Understanding the profound impact of such transitions enables you to provide culturally sensitive and responsive care to individuals from diverse backgrounds.

• A Master’s degree in Social Work (MSW) from an accredited program is required.
• A valid and active license to practice as a Clinical Social Worker (LCSW) in the state of Georgia.
• Exceptional communication and interpersonal skills, with the ability to convey empathy and understanding.
• Capacity to work both independently and collaboratively as a member of a healthcare team.
• Strong ethics regarding confidentiality and the handling of sensitive information.

• Experience working in hospice, palliative care, or similar healthcare settings is highly regarded.
• Familiarity with trauma-informed care principles and grief counseling techniques.
• Proficient in using electronic health record (EHR) systems to document client interactions and progress.
• Prior experience collaborating with healthcare providers to develop comprehensive care plans.
