• Conduct thorough psychosocial assessments to identify clients’ unique mental health needs, strengths, and available resources.
• Provide individual and family therapy, utilizing evidence-based methodologies such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop and implement personalized treatment plans, establishing clear goals and effective interventions tailored to each client.
• Collaborate closely with psychiatrists, nurses, and other healthcare professionals to ensure comprehensive and coordinated care.
• Facilitate group therapy sessions, psychoeducation classes, and support groups for families and patients experiencing grief and loss.
• Offer crisis intervention services, conducting risk assessments and safety planning whenever necessary.
• Maintain precise and timely documentation to adhere to ethical standards and legal requirements, ensuring transparency and accountability in care delivery.
• Participate in regular team meetings to discuss client progress and adapt treatment plans based on evolving needs.

In this role, you will focus on a diverse clientele, particularly individuals facing terminal illnesses and their families who are undergoing significant emotional distress. Your work will ensure that clients receive the mental health support they need during one of the most challenging times in their lives, promoting resilience and coping strategies to alleviate anxiety and emotional pain.

• A Master’s degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant clinical experience in a mental health or hospice setting.
• Proficiency in evidence-based therapeutic techniques and a strong foundation in psychosocial assessments.
• Exceptional communication, interpersonal, and assessment skills to build rapport with clients and families.

• Experience with end-of-life issues and grief counseling is highly desirable.
• Familiarity with digital platforms for telehealth services.
• Ability to work collaboratively within multidisciplinary teams while maintaining a client-centered approach.
