• Conduct comprehensive psychosocial assessments that thoroughly evaluate the strengths and needs of clients to determine their unique support requirements.
• Provide evidence-based therapeutic services, including individual, family, and group counseling, tailored to the diverse needs of our clients.
• Collaborate with healthcare providers, including nurses and physicians, to create and execute integrated care plans that address both mental health and physical well-being.
• Advocate for clients' rights by facilitating access to resources such as community support services, financial assistance, and healthcare programs.
• Maintain accurate documentation of client interactions, treatment progress, and outcomes within our electronic health record systems.
• Engage in ongoing training and attend team meetings to continue building clinical skills, learn new evidence-based practices, and stay current with advancements in social work.

Your primary client base will include individuals and families engaged with our hospice center, focused on navigating end-of-life transitions and processing grief. You will also work with clients from various backgrounds, each facing unique challenges that require a tailored approach to mental health support. Compassion, understanding, and cultural competence are critical as you engage with those experiencing sensitive emotional states.

• A Master’s degree in Social Work from an accredited institution.
• Active and current licensure as a Licensed Clinical Social Worker (LCSW) in California.
• A minimum of two years of post-licensure experience in clinical social work or related fields, demonstrating a strong foundation in mental health care.
• Capabilities in conducting diagnostic assessments and developing comprehensive treatment plans effectively.
• Outstanding interpersonal and communication skills that enable rapport-building with a diverse clientele.
• A commitment to cultural competence, social justice, and maintaining high ethical standards in your practice.

• Experience working with hospice or palliative care settings is highly desirable.
• Familiarity with telehealth platforms and remote service delivery methodologies.
• Knowledge of local resources for underserved populations, including healthcare, legal, and financial assistance.
