This position is fully remote, offering flexibility to conduct consultations and therapy sessions from the comfort of your home. As part of a community health clinic, you will be integrating clinical expertise with the convenience of telehealth services, allowing you to reach clients effectively and efficiently. The standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend duties as client needs dictate. This role will require adaptability in scheduling to ensure comprehensive care and on-call availability to address urgent situations.

• Conduct comprehensive assessments of clients' psychosocial needs, utilizing evidence-based tools to evaluate emotional, mental, and social factors affecting overall well-being.
• Collaborate closely with clients and their families to create tailored care plans that align with individual preferences and goals, ensuring an active role in their care process.
• Provide ongoing counseling and emotional support to seniors, navigating life transitions, such as relocation to assisted living or coping with loss and grief.
• Advocate for clients by ensuring they have access to appropriate community resources and services, effectively addressing their rights and needs.
• Work collaboratively with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to enhance the quality of care provided to clients.
• Carry out regular check-ins and virtual visits to monitor clients’ health and safety, identifying any changes that require immediate attention.
• Facilitate effective communication among family members involved in the senior client's care, resolving conflicts and improving coordination of services.
• Educate clients and their families about available community resources, support groups, and programs to promote social engagement and overall quality of life.
• Maintain meticulous documentation of client interactions, progress notes, assessments, and care plans while adhering to HIPAA regulations and ethical standards.
• Stay engaged with current research and developments in social work practices and policies to continuously enhance care effectiveness.

This position primarily focuses on the senior population, which involves addressing unique challenges such as mental health issues related to aging, social isolation, chronic illnesses, and the complexities of familial roles as caregivers. Your expertise will directly contribute to improving the lives of seniors, enhancing their emotional resilience and overall health outcomes in a supportive, respectful manner.

• Master's degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Clinical Social Worker (LCSW) is mandatory.
• A minimum of two years of experience in a social work role, particularly in community health or geriatric settings, is preferred.
• Strong understanding of social work principles, community resources, and aging-related issues.
• Exceptional assessment, counseling, and problem-solving skills.
• Demonstrated ability to empathize with and support seniors and their families, building trusting relationships.
• Excellent verbal and written communication skills, facilitating effective teamwork with healthcare professionals.
• Proficient in using electronic health record (EHR) systems and basic computer applications.

While a minimum of two years of social work experience is required, candidates with experience specifically in mental health counseling for seniors and knowledge of geriatric psychology will be highly favored. Familiarity with remote service delivery models and telehealth platforms will also set candidates apart in this role.
