• Conduct thorough clinical assessments to identify the mental health needs of patients.
• Develop individualized treatment plans employing evidence-based therapeutic strategies tailored to each patient.
• Utilize solution-focused therapy techniques in therapy sessions to empower patients and promote effective coping mechanisms.
• Maintain accurate, detailed clinical documentation in compliance with healthcare standards, ensuring timely updates and confidentiality.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other therapists, to ensure comprehensive care.
• Educate and support patients and their families about available resources, treatment options, and coping strategies.
• Engage in professional development by staying current with research and new methodologies in mental health treatment.

You will be working closely with a diverse patient demographic, focusing primarily on individuals facing terminal illnesses and their families. This role emphasizes the importance of addressing psychosocial issues while providing emotional support and practical guidance during a critical time in the patient’s life. Understanding the unique needs of hospice patients and respecting their wishes and values are essential in delivering compassionate care.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of New York.
• Strong understanding and experience with solution-based psychotherapy interventions.
• Exceptional verbal and written communication skills, coupled with proficient problem-solving abilities.

Experience in hospice care or end-of-life counseling is highly favorable. Familiarity with integrative approaches to healthcare and the ability to navigate the complexities of grief and bereavement counseling will be an asset in this role.

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience.
