This position is primarily remote, which facilitates accessibility and convenience for both our clinical staff and the families we serve. • Conduct thorough assessments of patients’ psychosocial needs, utilizing standardized tools and interviews to develop individualized treatment plans.
• Implement evidence-based therapeutic interventions, such as cognitive behavioral therapy and mindfulness-based practices, tailored to each client’s unique circumstances.
• Maintain detailed and confidential clinical records in adherence to HIPAA regulations, ensuring accuracy and clarity in patient documentation.
• Work collaboratively with a multi-disciplinary treatment team, including physicians, nurses, and other healthcare professionals, to provide a holistic approach to patient care.
• Educate clients and their families about mental health conditions and treatment options, empowering them to engage actively in their care and recovery journey.
• Continuously update and enhance your clinical knowledge by participating in professional development opportunities, workshops, and staying informed on current research in mental health.

In this role, you will provide essential support to patients and families navigating the complexities of hospice care. This includes working with individuals facing end-of-life issues, chronic illnesses, and emotional challenges associated with grief and loss. Your expertise and compassionate approach will ensure that our clients receive the emotional, social, and spiritual support necessary to enhance their quality of life during these critical moments.

• A Master’s degree in Social Work from an accredited institution.
• Active LCSW license in California.
• Minimum of 2 years of clinical experience, preferably within a hospice or end-of-life care setting.
• Expertise in evidence-based therapeutic interventions and familiarity with an array of psychosocial treatment methodologies.
• Strong interpersonal and communication skills with the ability to build trusting relationships with clients and their families.
• Exceptional problem-solving abilities and the capacity to think critically in challenging situations.
• Experience working collaboratively within a healthcare team, demonstrating flexibility and adaptability.

• Experience in palliative care or hospice settings, addressing the unique psychological and emotional needs of terminally ill patients.
• Familiarity with case management processes and community resources available to support patients and families.
• Previous involvement in patient advocacy initiatives to improve mental health outcomes.

This role offers a competitive salary range of USD 120,000 to 130,000 annually, along with an extensive benefits package that includes:
