• Conducting thorough clinical assessments to evaluate the mental health needs of patients in a hospice setting, developing personalized treatment plans that reflect their unique circumstances and preferences.
• Delivering comprehensive psychotherapy services using a variety of evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT) and solution-focused therapy, tailored to meet the emotional needs of patients and their families.
• Documenting clinical interactions and maintaining up-to-date patient records in accordance with HIPAA standards, ensuring confidentiality and compliance with regulatory requirements.
• Collaborating with an interdisciplinary team of healthcare professionals, including nurses and physicians, to coordinate care and create a supportive network for patients during their end-of-life journey.
• Engaging in case consultations and peer reviews to continually enhance treatment strategies and uphold the standards of care within our practice.
• Providing psychoeducation and emotional support to both patients and their families, equipping them with the tools to cope with mental health challenges associated with serious illness.
• Staying informed about current research in mental health and hospice care, integrating new knowledge into practice for improved patient outcomes.

This role focuses on providing care to patients facing terminal illnesses, where you will help address the complex emotional and psychological challenges they may encounter. Your expertise will support families navigating grief, transition, and end-of-life decisions, providing critical emotional care during this profoundly impactful time.

• A Master’s degree in Social Work from an accredited program.
• An active and valid Licensed Clinical Social Worker (LCSW) license in Florida.
• A minimum of three years of experience in a clinical setting, with a demonstrable focus on providing psychotherapy services.
• Understanding of solution-based interventions and strong competency in crisis intervention approaches.
• Excellent communication, empathetic listening, and interpersonal skills to connect effectively with patients and families.
• Ability to utilize telehealth systems proficiently, adapting to a remote practice effectively.
• A commitment to upholding ethical standards set by the National Association of Social Workers (NASW).

Experience in hospice or palliative care environments is highly desirable, as well as proficiency in addressing end-of-life emotional concerns. Familiarity with the unique needs of patients with terminal diagnoses and their families will enhance your effectiveness in this role.
