This position is structured as a remote role, allowing flexibility and the comfort of working from home while servicing patients primarily through virtual consultations. The team collaborates digitally, utilizing a variety of telehealth technologies to maintain effective communication and support. Each social worker has the autonomy to manage their schedule while adhering to standard work hours from Monday to Friday, 9:00 AM to 5:00 PM.

• Provide high-quality psychotherapy and mental health services to hospice patients dealing with conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive mental health assessments to evaluate patients' emotional, behavioral, and social functioning as part of their overall care plan.
• Develop personalized treatment plans that are evidence-based, culturally sensitive, and tailored to the specific needs of each patient.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to coordinate care and ensure holistic treatment approaches.
• Monitor and document patient progress, adjusting treatment plans as needed based on their feedback and clinical responses.
• Provide essential education and support to patients and their families about mental health conditions, treatment options, and community resources available for additional assistance.
• Adhere to all ethical guidelines and professional standards, maintaining confidentiality and obtaining informed consent as necessary.

The successful LCSW will primarily work with patients within a hospice setting, facing terminal illnesses or complicated grief and emotional issues surrounding end-of-life care. This role requires cultural competency and sensitivity, as you will support patients from diverse backgrounds, ensuring they receive compassionate care tailored to their unique needs and beliefs.

• A master’s degree in Social Work from an accredited institution, ensuring advanced clinical training and education.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
• A minimum of two years of experience in providing psychotherapy and mental health services, particularly in hospice or palliative care.
• Exceptional communication and interpersonal skills to establish trust with patients and collaborate effectively with a healthcare team.
