Location: Fully remote (Never coming Onsite)
Schedule: Hiring for a M-F 8 hour shift starting for 9: 00AM ET / 8: 00 AM CT. Training will be 8: 00a-4: 30p CT
*WFH - Must be located in a participating, Compact Licensure state and have an active, in good standing nurse license in that state.
States to be excluded: California, New York, Illinois, Massachusetts (awaiting implementation) Michigan, Minnesota, Alaska and Hawaii.
MUST HAVE:

1. MTM Delivery Experience (Depth Matters)
- Hands on experience delivering telephonic comprehensive medication reviews
- Ability to identify, document, and resolve medication related problems
- Experience communicating recommendations to prescribers
2. Compact RN Licensure + State Eligibility
- Active RN license in a Compact Licensure state
- Ability to legally practice telephonically across states
3. Call Center / Telephonic Patient Engagement
- Comfort handling back to back inbound/outbound calls
- Strong documentation while on calls
- Behavioral interviewing / motivational interviewing skills
Duties:
1. Perform telephonic comprehensive medication reviews, which includes but is not limited to ensuring a complete medication reconciliation is completed and performing clinical assessments on applicable conditions for each targeted member.
2. Identify, document, and facilitate resolution of identified medication related problems.
3. Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy.
4. Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems.
5. Build and maintain positive relationships with internal and external customers including physicians, pharmacists, nurses and coordinators.
6. Other responsibilities as directed by supervisor and/or manger.
Experience:
Required:
Associate's Degree in Nursing AND Compact Licensure in State of Practice
• 2 years minimum of related practice experience
Applicant must reside and have an active nurse license in Compact Licensure State.
Preferred:
Bachelor's Degree in Nursing
Case Care Management Certificate
Previous experience delivering MTM, specifically using the Outcomes MTM Connect platform.
Strong clinical knowledge and understanding of chronic diseases, preventative care, guideline driven recommendations, and health promotion.
Basic computer skills and proficiency with Microsoft Office products.
Strong organizational skill, interpersonal skills, and detail orientated.
Experience maintaining focus and meeting goals in a work from home environment.
Previous call center experience
Position Summary:
Medication Therapy Management supports the company vision to help people on their path to better health by performing MTM for members at the highest-risk of having medication related problems. The team of clinicians, technicians, and coordinators work together to identify problems and coordinate care to facilitate resolution to ensure each member is on an optimized medication regimen to help them reach their health-related goals.
The role of the MTM Nurse is to deliver telephonic medication therapy management (MTM). Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Nurse interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health-related goals and help them on their path to better health. Nurses will also be trained to use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes.
