Job description

GoTo Telemed is seeking board-certified Health & Wellness Coaches (NBC-HWC) to join our nationwide digital health enterprise. In this 100% remote clinical role, you will partner with our patients to facilitate sustainable lifestyle changes, manage chronic conditions, and promote whole-person wellness.

Operating within our integrated ecosystem, you will not work in a silo. You will collaborate daily with our primary care physicians, virtual behavioral specialists, and remote dental teams. As a Virtual Clinical Health Coach, you will serve as the longitudinal behavioral bridge between a patient's medical diagnosis and their daily lifestyle implementation, utilizing evidence-based behavioral change methodologies without the pressure of physical practice administration or patient acquisition.

Key Responsibilities
  • Behavioral Change Facilitation: Conduct live, synchronous video and audio sessions using Motivational Interviewing (MI) and cognitive-behavioral techniques to help patients establish and achieve personalized health goals.
  • Chronic Care Management (CCM): Serve as a core team member in our CCM and Remote Patient Monitoring (RPM) programs, assisting patients in managing chronic conditions such as diabetes, hypertension, and obesity through lifestyle modifications.
  • Clinical Health Coaching: Deliver structured, evidence-based health and well-being coaching sessions (tracking encounters via CPT codes 0591T, 0592T, and 0593T).
  • Interdisciplinary Care Coordination: Review provider treatment plans and translate clinical recommendations (e.g., dietary changes, exercise protocols, stress reduction) into actionable, daily steps for the patient.
  • Patient Triage & Escalation: Monitor patient progress and self-reported metrics; rapidly identify acute physical or psychological symptoms and escalate cases to the appropriate licensed GoTo Telemed diagnosing provider (MD, DO, NP, or LCSW).
  • Documentation & Compliance: Accurately document all coaching interventions, goal progression, and patient interactions in the GoTo Telemed cloud EHR within 24 hours, adhering strictly to HIPAA and enterprise recordkeeping standards.
Minimum Qualifications & Requirements
  • Certification: Must hold an active, unencumbered National Board Certified Health & Wellness Coach (NBC-HWC) credential. (Note: Non-accredited or purely commercial coaching certificates will not be accepted for this clinical role).
  • Education: Bachelor's degree or higher in Health Education, Kinesiology, Nutrition, Psychology, Nursing, or a related clinical field.
  • Experience: Minimum of two (2) years of experience providing direct patient/client health coaching, preferably within a clinical, healthcare, or telehealth setting.
  • Scope Awareness: Deep understanding of the legal scope of practice for health coaches, recognizing the clear boundary between coaching, medical diagnosing, and registered dietetics.
  • Digital Literacy: High proficiency in navigating cloud-based Electronic Health Records (EHR), telehealth video suites, and secure messaging portals.
  • Workspace: A dedicated, private, and acoustically sound home office space equipped with high-speed internet, a high-definition webcam, and a headset to guarantee HIPAA-compliant privacy.
