The Cohesive Family Psychotherapy Center is seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our remote outpatient team. This full-time role involves delivering comprehensive mental health services to clients within a supportive and collaborative environment. This position offers the flexibility of remote work, with an outpatient facility in Santa Rosa serving as your primary base. Your working hours will typically be Monday through Friday, from 8 AM to 5 PM. While you will primarily operate from home, occasional team meetings or training sessions may require your presence at our Santa Rosa office, fostering team connection and collaboration.

• Conduct thorough assessments and develop individualized care plans for clients experiencing major mental illness or severe emotional challenges, ensuring a tailored treatment approach.
• Engage collaboratively with other team members to effectively meet shared goals, emphasizing strong communication and mutual support.
• Provide culturally competent care, respecting diverse backgrounds and adapting strategies to each client’s unique interpersonal dynamics.
• Adhere to professional ethical guidelines, ensuring that all interventions align with best practices and regulatory standards.
• Facilitate therapy sessions—individually, in groups, or with families—to empower clients during their recovery journey.
• Utilize evidence-based practices for screening, diagnosis, and treatment, adjusting approaches as necessary based on client needs and progress.
• Monitor and document client progress, ensuring timely completion of reports that contribute to overall team performance evaluations.
• Respond to crisis situations effectively, utilizing intervention skills to support safety and stability for clients in distress.
• Participate in quality improvement committees aimed at enhancing service delivery and operational efficiencies within the agency.
• Maintain on-call availability as needed, ensuring support for clients during and after program hours.

You will work with a diverse client base in a remote setting, primarily focusing on individuals suffering from complex mental health conditions and trauma. Your role will involve working with patients from different backgrounds, including those in economically disadvantaged situations. This work is critical to ensuring equitable access to mental health treatment, and your contributions will directly impact the lives of individuals and families in need.

• A Master’s degree in Social Work (MSW) or a related field is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California is necessary.
• Demonstrated expertise in assessment, diagnosis, and treatment planning for various populations.
• Exceptional verbal and written communication skills, facilitating strong interactions with clients and team members alike.
• Strong organizational abilities, enabling you to manage a full caseload effectively while maintaining quality standards.
• Proven competency in addressing age-related or specialty issues, ensuring sensitive care across the lifespan.

• A minimum of 2 years of clinical experience is preferred, particularly in settings serving underserved populations.
• Bilingual in English and Spanish is highly preferred, enhancing communication with a broader client demographic.

The salary for this full-time position ranges from USD 70,000 to 85,000 annually, commensurate with experience and education.
