In this primarily remote role, you will have the flexibility to manage your schedule while being an integral part of a compassionate hospice team. Collaborating closely with healthcare providers including physicians, nurses, and other mental health specialists, you will engage in regular virtual meetings, consultations, and case reviews to ensure comprehensive care planning. • Conduct thorough psychosocial assessments to evaluate the mental health status, strengths, and challenges of hospice patients.
• Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) or dialectical behavior therapy (DBT), to promote emotional well-being.
• Collaborate with a multidisciplinary team, including physicians, nurses, and chaplains, to coordinate comprehensive care plans that address both physical and emotional needs.
• Support patients and their families in understanding diagnoses and treatment options, guiding them through complex healthcare decisions.
• Facilitate individual, group, and family therapy sessions, utilizing evidence-based practices tailored to the hospice setting.
• Develop and implement personalized treatment plans that align with patient goals and involve contributions from the treatment team.
• Advocate for patients' rights and needs within the healthcare system, assisting them in navigating resources and support services.
• Lead psychoeducational sessions and therapeutic groups to promote coping strategies and resilience among patients and families facing terminal illnesses.

The position primarily involves working with terminally ill patients and their families, providing emotional support that addresses the psychological, social, and spiritual facets of end-of-life issues. Your role will directly contribute to helping patients cope with their diagnoses and ensure that their final days are filled with dignity, peace, and support.

• Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
• Demonstrated knowledge of mental health disorders, treatment modalities, and crisis intervention techniques, particularly in hospice and palliative care contexts.
• Proficiency in utilizing electronic health record (EHR) systems for documentation and coordination of care.
• Strong interpersonal and communication skills to work collaboratively within a multidisciplinary team.

• Experience working in hospice, palliative care, or medical social work settings is strongly preferred.
• Familiarity with community resources for end-of-life care and support services.
• Experience in providing grief counseling or bereavement support is a plus.
