• Conduct detailed assessments of client situations to identify strengths, needs, and challenges, and use this information to develop personalized treatment plans.
• Implement evidence-based therapeutic interventions that promote positive mental health outcomes, including cognitive-behavioral therapy and solution-focused techniques.
• Maintain thorough and confidential records in accordance with legal and ethical standards, ensuring that client information is safeguarded.
• Collaborate with a multidisciplinary treatment team, engaging in regular case discussions to enhance care and ensure client-centered approaches.
• Stay informed about the latest research, tools, and resources in social work practice to continually improve the therapeutic process.

You will work primarily with patients in a hospice setting who may experience complex emotional and psychological issues, including trauma, anxiety, and depression. Your role will involve providing empathetic support to both patients and their families, guiding them through the end-of-life process and facilitating conversations about grief, loss, and transition.

• A Master's degree in Social Work from an accredited institution is required for this position.
• Possession of an active Licensed Clinical Social Worker (LCSW) license in the state of Texas is mandatory.
• Strong communication, interpersonal, and problem-solving skills are essential for effective collaboration with clients and team members.
• The ability to thrive in a remote work environment and demonstrate self-motivation in managing caseloads effectively.

• Experience providing therapeutic support in a hospice or end-of-life care setting is highly desirable.
• Knowledge of trauma-informed care practices and evidence-based therapeutic modalities.
• Prior experience working with diverse patient populations, particularly in addressing mental health issues like anxiety and depression.
