Key Responsibilities

• Collaborate with primary care practitioners to identify, diagnose, and manage mental health and psychosocial challenges, enhancing the overall efficacy of the patient care team.
• Conduct comprehensive patient intakes, focusing on diagnostic and functional assessments, followed by the development of targeted therapeutic objectives and treatment recommendations.
• Provide timely and focused emergency mental health interventions to patients in crisis.
• Advise primary care providers on optimal treatment pathways, recommending referral to specialized mental health services when necessary.
• Conduct regular follow-up evaluations to monitor client progress and adjust treatment plans as required.
• Assist patients in developing relapse prevention strategies and actively support their adherence to these strategies.
• Identify patients at risk of escalating health issues and contribute to the development of preventative care programs tailored to their needs.
• Document patient progress and treatment plans thoroughly in electronic health records, ensuring compliance with Agency and Federally Qualified Health Center (FQHC) standards.
• Maintain ongoing communication with primary care providers regarding patient status and collaborate on treatment planning.
• Encourage behavioral change in patients with associated physical ailments by promoting healthier lifestyle choices.
• Provide essential self-management skills and educational resources to empower patients in their treatment journeys.
• Consult with clinic management and other team members on behavioral health services, identifying opportunities for program enhancement and outcome evaluation.
• Ensure compliance with ethical codes and standards expected of clinical professionals.
• Undertake additional assignments as required to support clinic operations.

In this role, you will serve a diverse patient population, including individuals seeking support for anxiety, depression, substance use disorders, and various psychosocial issues. Qualifications

• A Master's Degree in Clinical Counseling is required.
• Active and valid LPC (Licensed Professional Counselor) license.
• Demonstrated knowledge of mental health assessment, treatment planning, and behavioral therapy methodologies.
• Ability to conduct psychodiagnostic evaluations using DSM-IV criteria.
• Strong communication skills, with bilingual proficiency in English and Spanish preferred.
• Excellent understanding of the community resources available to patients and insight into their utilization.
• Capacity to analyze individual patient needs and tailor treatment accordingly.
• Well-rounded mental and physical health to perform the duties of the role effectively.

Preferred Experience

• Prior experience in a community health or outpatient clinic setting is beneficial.
• Proven track record of collaborative work in integrated care teams.
• Experience in crisis intervention and emergency counseling.
• Familiarity with electronic health record systems and clinical documentation requirements.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 90,000 to 105,000, commensurate with experience, alongside a SIGN-ON BONUS. Additional benefits include comprehensive health coverage, flexible work hours, and opportunities for professional development.
