As a member of our Hospice Center team, you will work alongside interdisciplinary professionals including physicians, nurses, and chaplains, promoting holistic approaches to patient care and emotional well-being.

• Conduct thorough psychosocial assessments to understand clients' mental health needs, strengths, and available resources.
• Provide individual, family, and group therapy sessions utilizing a variety of evidence-based treatment modalities, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed approaches.
• Create and implement tailored treatment plans with clearly defined goals and interventions to optimize patient outcomes.
• Collaborate closely with psychiatrists, psychologists, and other healthcare professionals to ensure a coordinated, multidisciplinary approach to client care.
• Facilitate group therapy, psychoeducation classes, and support groups, helping clients to navigate their challenges in a supportive community setting.
• Provide crisis intervention and develop safety plans tailored to the unique needs of clients facing acute psychological distress.
• Maintain comprehensive and coherent documentation that adheres to ethical and legal standards, ensuring clarity and continuity of care.

In this role, you will work primarily with clients in the hospice setting, where individuals may be experiencing grief, fear, depression, or anxiety surrounding impending life changes. Your support will be instrumental in helping them process these emotions and find meaning in their experiences, all while addressing their mental health needs with sensitivity and professionalism.

• A Master’s degree in Social Work from an accredited program.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• A minimum of two years of relevant experience in a mental health or outpatient context, preferably involving end-of-life care or chronic illness.
• In-depth knowledge of evidence-based treatment practices and a strong grasp of psychotherapeutic methodologies.
• Exceptional interpersonal, communications, and assessment skills, with a focus on patient-centered care.

• Experience working with hospice or palliative care populations and a clear understanding of the psychological aspects of chronic illness.
• Familiarity with electronic health records (EHR) and telehealth platforms for remote service delivery.
• Prior involvement in interdisciplinary teams, enhancing collaborative approaches to treatment.
