This full-time role operates in a remote setting, allowing for a flexible work-life balance while maintaining a strong connection to the school’s healthcare team. As a member of the Social Work Services department, you will collaborate with educators, healthcare providers, and families, keeping the focus on delivering integrated and patient-centered care.

• Manage a designated caseload of students by evaluating their social, emotional, financial, and cultural factors that impact their overall health and educational experience. Utilize a comprehensive approach to develop tailored therapeutic interventions.
• Lead interdisciplinary coordination efforts to tackle issues such as protective services, life care decisions, and advance directives by facilitating discussions and ensuring all team members are aligned on complex treatment plans.
• Work closely with case managers to provide education, resources, and appropriate referrals to students and families, assuring that psychosocial and socioeconomic needs are prominently integrated into their treatment or discharge plans.
• Supervise social work interns and newly licensed staff, offering guidance and support while promoting best practices in social work at the school level.
• Develop and maintain community partnerships to ensure students and families have access to necessary resources, aiming to minimize hospital readmission through strategic care coordination.
• Respond promptly and effectively to psychosocial service requests, ensuring the timely implementation of treatment plans and facilitating smooth transitions out of crisis situations when they arise.
• Participate in regular team meetings and care conferences to promote collaborative care planning and integrate feedback into ongoing treatment strategies.

This role primarily focuses on school-aged children and adolescents facing various psychosocial issues. You will interact closely with students from diverse backgrounds, addressing their unique mental health challenges, family dynamics, and social environments to promote their well-being and academic success.

• A Bachelor’s Degree in Social Work is required.
• A current and valid Licensed Clinical Social Worker (LCSW) credential.
• Minimum of 2 years of relevant experience, ideally within a healthcare or educational setting.
• Proficient in case management, crisis intervention, and developing treatment plans tailored to individual needs.
• Strong interpersonal and communication skills, with the ability to work collaboratively with multidisciplinary teams.

• Experience working within school systems or with pediatric populations.
• Familiarity with community resources and referrals applicable to families and children.
• A proven track record of enhancing patient outcomes through targeted interventions and advocacy.

This position offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
