Key Responsibilities
  • Manage a designated caseload of students by evaluating their social, emotional, financial, and cultural factors that impact their overall health and educational experience. Utilize a comprehensive approach to develop tailored therapeutic interventions.
  • Lead interdisciplinary coordination efforts to tackle issues such as protective services, life care decisions, and advance directives by facilitating discussions and ensuring all team members are aligned on complex treatment plans.
  • Work closely with case managers to provide education, resources, and appropriate referrals to students and families, assuring that psychosocial and socioeconomic needs are prominently integrated into their treatment or discharge plans.
  • Supervise social work interns and newly licensed staff, offering guidance and support while promoting best practices in social work at the school level.
  • Develop and maintain community partnerships to ensure students and families have access to necessary resources, aiming to minimize hospital readmission through strategic care coordination.
  • Respond promptly and effectively to psychosocial service requests, ensuring the timely implementation of treatment plans and facilitating smooth transitions out of crisis situations when they arise.
