Key Responsibilities

  • Deliver individual and group counseling services, focusing on crisis intervention, treatment planning, and diagnostic evaluations tailored to patients and their families.
  • Document patient interactions and progress notes electronically using our case management system, ensuring compliance with healthcare regulations.
  • Collaborate with healthcare providers to escalate patient needs effectively, facilitating appropriate care pathways and referrals.
  • Serve as a liaison between patients and community resources, assisting with access to medication assistance programs and support services.
  • Work with Quality Improvement teams to streamline data tracking and improve service delivery based on patient feedback.
Patient Population / Client Focus

The primary patient population consists of individuals and families facing terminal illness, requiring compassionate mental health support and assistance in coping with grief, anxiety, and emotional distress. Your role will be integral to improving the quality of life for these clients through empathetic listening, effective counseling strategies, and individualized treatment plans.

Qualifications
  • Possession of a valid Licensed Clinical Social Worker (LCSW) license in the state of New York.
  • A Masterโ€™s degree in Social Work is required, complemented by a minimum of three (3) years of post-licensure experience in a related field.
  • Strong understanding of therapeutic techniques and crisis intervention strategies, coupled with the ability to make independent judgments regarding client care.
  • Proficient in technology, particularly with the use of electronic health records and telehealth systems to manage client information.
