• Conduct comprehensive patient intakes by performing detailed diagnostic assessments and functional evaluations, providing recommendations for treatment plans to the primary care team.
• Deliver brief, focused therapeutic interventions tailored to the individual needs of patients requiring mental health support.
• Collaborate with primary care providers to identify patients' mental health and psychosocial issues, ensuring proper management within a coordinated care framework.
• Monitor and document patient progress diligently in treatment charts while keeping the primary care team informed to facilitate integrated treatment strategies.
• Design and implement relapse prevention plans as necessary, assisting patients in maintaining their mental health and functional status.
• Detect ‘at risk’ patients and develop proactive plans to prevent the exacerbation of their mental health conditions.
• Help patients navigate the complexities of medical treatment initiated by primary care providers, offering support with coping strategies for medication side effects.
• Provide consultations to clinic leadership and fellow team members on behavioral health services, contributing to program evaluation and outcome improvement efforts.
• Adhere strictly to ethical guidelines and the Code of Ethical Conduct for Clinical Professionals while maintaining the highest standards of patient confidentiality.
• Carry out additional duties as assigned to ensure comprehensive support for mental health services within the clinic.

As an LPC at The Core For Relationship Healing, you will work with a diverse client base, including individuals and families dealing with a variety of mental health challenges. • A minimum of a Master’s Degree in Clinical Counseling from an accredited institution.
• A valid and current LPC (Licensed Professional Counselor) license.
• In-depth knowledge of mental health assessment standards, treatment planning, and various therapeutic modalities, including behavioral therapy, family therapy, and child psychotherapy.
• Proficiency in English is required; bilingual proficiency in English and Spanish is preferred but not mandatory.
• Strong understanding of the psychological and social impacts of mental health issues on individuals and their families.
• Ability to maintain comprehensive clinical records and prepare insightful reports for interdisciplinary team discussions.

• Experience in a community health clinic or a multidisciplinary healthcare environment is highly valued.
• Familiarity with treatment protocols addressing the unique mental health needs of diverse populations.
• Experience in conducting telehealth services and familiarity with relevant technology platforms.

We offer a competitive salary ranging from USD 87,000 to 105,000 annually, based on experience and qualifications. In addition, candidates are eligible for a sign-on bonus.
