• Collaborate with primary care providers to identify, assess, and manage mental health and psychosocial issues, contributing as an integral member of the healthcare team.
• Conduct comprehensive patient intakes, focusing on thorough diagnostic evaluations and functional assessments, to inform treatment goals and care plans.
• Provide brief yet impactful interventions for patients seeking mental health services, addressing immediate needs and emotional crises.
• Evaluate treatment plans recommend referrals when specialty care is more appropriate, ensuring that patients receive the best possible support.
• Monitor patient progress through follow-up sessions, adjusting treatment strategies as warranted based on their evolving needs.
• Develop and implement relapse prevention plans tailored to individual circumstances, aiding patients in achieving and maintaining stability in their lives.
• Identify at-risk patients and actively participate in developing proactive plans to prevent deterioration of their mental health.
• Coordinate and link patients with other healthcare providers, ensuring comprehensive behavioral health services.
• Maintain accurate and thorough documentation of patient progress and treatment plans within our electronic health records system.
• Educate patients on self-management strategies and healthy lifestyle choices, empowering them to engage fully in their recovery.
• Assist clinic management by providing insights into behavioral services and outcomes, contributing to the overall effectiveness and evaluation of our programs.
• Support patients in adhering to medical treatments prescribed by primary care providers, including strategies for managing side effects from medications.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals, balancing professionalism with compassionate care.

In this role, you will work closely with a diverse patient population, addressing various mental health issues, including anxiety, depression, trauma, and substance use disorders. Your expertise will be pivotal in fostering a safe, supportive environment that encourages patients to confront their challenges and work towards recovery.

• A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
• A current and valid LPC (Licensed Professional Counselor) license is mandatory to practice.
• Demonstrated knowledge of mental health assessment techniques, treatment planning, and developmental psychopathology.
• Understanding of family therapy, group therapy, and child psychotherapy methodologies, as well as DSM-IV diagnoses.
• Preferred proficiency in both English and Spanish to effectively communicate with our diverse clientele.
• Your mental and physical health must be sufficient to handle the demands of the position.
• A comprehensive understanding of the social and personal effects of mental health issues on individuals and communities is crucial.
• Familiarity with community resources and support systems that can assist patients in their recovery journey.

• Experience in an outpatient clinical setting, preferably within a primary care framework, is preferred.
• Proven ability to work collaboratively in a team-oriented environment.
• Knowledge of psychodiagnostic techniques and psychotherapeutic approaches is a plus.

The role offers a competitive salary ranging from USD 87,000 to USD 105,000 annually, commensurate with experience. Additionally, we provide a sign-on bonus to welcome you to our team.
