Key Responsibilities
  • Assist primary care providers in identifying and managing mental health and psychosocial challenges, contributing to a holistic approach to patient care.
  • Conduct patient intakes focusing on diagnostic and functional evaluations, and make tailored recommendations to primary care providers regarding treatment goals and plans.
  • Implement brief, focused interventions for patients requiring mental health services, guiding them towards healthier lifestyle choices.
  • Initiate follow-up communications to track patient progress and adjust treatment methods as needed.
  • Develop relapse prevention plans and support patients in maintaining stable mental health.
  • Detect at-risk patients and collaborate on strategies to prevent further deterioration of their conditions.
  • Document patient progress and engage with primary care teams to keep them informed of patient needs and developments.
  • Assist patients in adhering to medical treatments prescribed by primary care providers, including helping manage any side effects of medications.
  • Provide consultation on behavioral health services to clinic management and participate in program evaluations aimed at improving outcomes.
  • Abide by the Code of Ethical Conduct for Clinical Professionals at all times.
