Key Responsibilities
  • Report directly to the Social Work Manager while evaluating, diagnosing, and treating a variety of psychosocial challenges faced by hospice patients and their families.
  • Utilize clinical social work skills to assist patients in overcoming personal and environmental barriers that hinder their psychosocial functioning.
  • Adhere to the NASW Code of Ethics and maintain professional standards of practice while delivering services.
  • Conduct comprehensive bio-psychosocial evaluations to identify treatment barriers and enhance patients' health status, leading to improved resource utilization.
  • Provide critical input on patients' psychosocial needs to medical and support staff, facilitating comprehensive care planning aimed at enhancing treatment compliance and coordination.
  • Develop and implement individualized treatment plans incorporating both individual and group therapeutic modalities, regularly analyzing treatment outcomes to optimize care.
  • Document social work activities and patient interactions in medical records in accordance with departmental standards.
  • Educate patients and families on medical and therapeutic recommendations, fostering understanding and adherence to create pathways for optimal recovery.
  • Advocate for patients and coordinate community resource accessibility to support their social needs, emphasizing client self-determination and empowerment.

Qualifications
