You will collaborate closely with a multidisciplinary team to ensure comprehensive, coordinated care. As a Licensed Clinical Social Worker, you will be responsible for:

• Conducting thorough bio-psychosocial evaluations to identify barriers to treatment and enhance patient health status.
• Developing and implementing individualized treatment plans based on clinical assessments and patient needs, utilizing both individual and group therapy modalities.
• Documenting all social work interventions in compliance with departmental standards and ensuring all patient records reflect the highest level of detail and accuracy.
• Educating patients and families on understanding care plans, medical conditions, and social work resources to ensure adherence to therapeutic recommendations.
• Advocating for patients’ needs within the community and coordinating advocacy activities to enhance access to social support services.
• Regularly collaborating with medical and support staff to develop integrated care strategies that improve overall patient satisfaction and health outcomes.
• Participating in continuous professional development and training to enhance clinical expertise, staying informed on community resources available to patients.

The LCSW will work primarily with diverse patient populations dealing with a variety of mental health challenges, including but not limited to depression, anxiety, trauma, and chronic illness. You will be tasked with addressing the unique psychosocial needs of each patient, considering their age, cultural background, and personal histories to tailor therapeutic approaches effectively.

Ideal candidates will possess:

• A Master’s Degree in Social Work from an accredited institution.
• A current and valid Licensed Clinical Social Worker (LCSW) license from the State Board of Social Work Licensure.
• Strong understanding of the NASW Code of Ethics and a commitment to adhering to professional standards of practice.

We seek individuals who have:

• A minimum of six months to one year of clinical experience, preferably in a health or mental health setting with exposed practice to the specific populations we serve.
• Experience in developing and implementing treatment plans, and employing therapeutic interventions.
• Knowledge of community resources with a proven ability to connect patients to necessary supports and services.
