In this role, you'll help deliver high-quality, member-centered care and play a key part in improving clinical outcomes, supporting care coordination, and enhancing the experience for both members and their caregivers. This opportunity is well suited to a compassionate medical assistant who is passionate about improving the health and well-being of older adults through personalized, holistic support.
Responsibilities
  • Review Community Health Worker (CHW) appointment documentation for accuracy and completeness, escalating issues to the nurse case manager as needed.
  • Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers.
  • Mail and/or email patient care plans and educational materials as directed by case managers.
  • Accurately update and maintain patient records within the electronic health record (EHR) system.
  • Protect patient confidentiality and support compliance with HIPAA and organizational policies.

Requirements
Experience, Skills, & Personality
  • Certified or Registered Medical Assistant (CMA/RMA) required.
  • 2 years of experience in a medical office, telehealth, or care coordination setting.
  • Experience using EHR systems; experience using eClinicalWorks preferred.
  • Experience working within HIPAA and organizational policy requirements in a healthcare setting.
  • Available during Eastern Time business hours.
  • Experience working with older adult populations preferred.
