In your role, you will:

• Conduct in-depth assessments of clients’ psychosocial needs, focusing on emotional, mental, and social factors impacting their health and quality of life.
• Develop tailored care plans in collaboration with clients and their families, ensuring alignment with their personal goals and preferences.
• Provide compassionate counseling to seniors and their families, aiding them through life transitions such as health changes, loss, or relocation.
• Act as a strong advocate for clients, helping them gain access to necessary resources and services within the community.
• Work closely with a multidisciplinary team including medical staff, caregivers, and other professionals to create a cohesive care strategy.
• Monitor clients’ situations through regular check-ins, adjusting care plans as needed to promote their health and well-being.
• Facilitate family meetings to improve communication and address conflict within the care process.
• Keep meticulous records of client interactions, assessments, and outcomes, maintaining compliance with our center’s policies and regulatory requirements.
• Stay current with social work best practices, ethical standards, and relevant laws to ensure the highest quality of care.

Your primary focus will be seniors within the community, who often face unique challenges including isolation, mental health issues, and difficulties accessing resources. You will help bridge the gap between their needs and the services available, thus enhancing their overall quality of life. • A Master’s degree in Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in Florida.
• A minimum of two years’ experience in a social work setting, with a preference for those who have worked with senior populations.
• A solid understanding of social work principles, community resources, and the particular challenges associated with aging.
• Exceptional communication, empathy, and interpersonal skills to effectively engage with clients and their families.
• Proficiency in electronic health record systems to maintain accurate documentation.

Ideal candidates will have experience in community health settings, with a focus on geriatric care. Skills in conflict resolution, crisis intervention, and resource mobilization are highly desirable. Additionally, candidates who have worked in remote environments or have experience with telehealth services will be given preference.
