Job description

Responsibilities

 Scribe and document physician encounters in real time or from recordings.

 Complete accurate, timely, and compliant medical documentation in the electronic

medical record (EMR).

 Document histories, physical exams, assessments, and treatment plans.

 Assist with progress notes, admissions, discharge summaries, and follow-up

documentation.

 Maintain HIPAA compliance and patient confidentiality.

 Work closely with the physician to ensure documentation is complete and accurate.

Qualifications

 Previous experience as a medical scribe or medical assistant preferred.

 Excellent English grammar, spelling, and typing skills.

 Strong understanding of medical terminology.

 Experience with EMR systems is preferred.

 Ability to work independently, prioritize tasks, and meet deadlines.

 Experience in skilled nursing, rehabilitation, long-term care, or post-acute medicine is a

plus.
