This position is designed for a licensed Clinical Social Worker who will work out of our Hospice Center, providing remote support to clients. You will be part of a collaborative team of mental health professionals, including psychiatrists and psychologists, focused on improving mental health outcomes and fostering a supportive atmosphere for clients and their families. • Conduct thorough psychosocial assessments to understand clients' mental health concerns and establish support mechanisms.
• Provide individual, family, and group therapy sessions utilizing evidence-based modalities such as cognitive-behavioral therapy, dialectical behavior therapy, and solution-focused therapy.
• Design and implement individualized treatment plans that include measurable goals and tailored interventions.
• Collaborate closely with a multidisciplinary treatment team to ensure a seamless continuum of care for each client.
• Facilitate psychoeducation classes and support groups to help clients develop coping strategies and resilience.
• Deliver crisis interventions and risk assessments to manage acute situations, ensuring the safety and well-being of clients.
• Maintain accurate clinical documentation in accordance with ethical standards and regulatory requirements.

This role primarily focuses on supporting clients with a variety of mental health issues, such as anxiety disorders, depression, trauma-related disorders, and adjustment challenges. Our patient population encompasses a wide range of individuals, including families navigating end-of-life issues, which requires sensitivity, empathy, and a strong commitment to enhancing the quality of patients’ mental health during critical times.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A Master's degree in Social Work from an accredited institution is required.
• Minimum of two years of experience in a mental health or outpatient setting, demonstrating hands-on experience with therapeutic practices.
• Proven knowledge of evidence-based therapeutic frameworks and the ability to apply them effectively in a clinical environment.
• Strong interpersonal skills, excellent communication abilities, and adept assessment skills are essential for effective client interactions.

• Experience working with diverse and underserved populations within a hospice or palliative care context is highly preferred.
• Background in community engagement and advocacy related to mental health awareness and support.
• Familiarity with telehealth platforms and virtual counseling techniques is an asset, given the remote nature of this position.
