This role is entirely remote, allowing you the flexibility to work from home while connecting with patients nationwide. Our telehealth setting is designed for modern mental health professionals, focusing on high-quality patient interactions through secure video conferencing platforms.

• Conduct thorough assessments of patients to determine their psychosocial needs, strengths, and areas where support is required, facilitating positive mental health outcomes.
• Provide therapeutic interventions tailored to individual patients, helping them navigate their mental health challenges and adjust positively to treatments, including dialysis.
• Deliver supportive counseling services, ensuring that all interventions are within your clinical training and licensed capabilities.
• Guide patients seeking transplant through educational and goal-directed counseling, assisting them in understanding the process and managing their expectations.
• Maintain an updated knowledge base of community resources and refer patients for additional support services like vocational rehabilitation and home health care to enhance treatment efficacy.
• Facilitate discussions regarding advance directives with patients and their families, ensuring their wishes are understood and incorporated into care plans.
• Collaborate actively with an interdisciplinary healthcare team to develop personalized care plans that reflect the unique needs and aspirations of each patient.
• Utilize established social work theories and tools throughout the counseling process, tracking quality-of-life measurements and barriers to help patients achieve their treatment goals.

Your focus will primarily be on individuals undergoing significant health challenges, such as those requiring regular dialysis or considering transplants. You will provide support to a diverse clientele, including patients with varying cultural backgrounds and life circumstances, ensuring a holistic approach to their mental health care.

• A valid social work license, such as LMHC, LMFT, or LICSW, as required by state regulations.
• A Master’s degree in Psychology, Social Work, Counseling, or a similar field.
• No history of medical malpractice claims, verified through a third-party background check.
• Authorization to work in the United States.

• Experience in a telehealth setting is preferred but not mandatory.
• Previous work with patients undergoing chronic medical treatments, such as dialysis or transplant care.
• Familiarity with community resources and vocational rehabilitation services.
