• Maintain accurate and timely documentation of clinical records and treatment progress to ensure compliance with healthcare regulations and facilitate effective communication within the care team.

The primary focus of this role is on a diverse population of patients receiving hospice care, along with their families. • A Master’s degree in Social Work from an accredited institution is required.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Illinois is essential.
